"Please raise my kids, Janet. You're the only one left I trust!"
The distraught King of Pop is convinced he won't survive to see his three children grow up if he has to battle the deadly disease that sweeps through its victims' bodies and consumes their flesh.
"Michael feels the end is near," a close family friend told The ENQUIRER. "He's terrified that if he dies, his children will end up with his aging parents or his brothers," a close family friend told The ENQUIRER.
"He knows Janet will give his kids a mother's love and see that they grow up away from the showbiz spotlight."
Michael, 50, is father to Prince Michael, 12, and Paris, 10 - both born to his ex-wife Debbie Rowe. Six-year-old Prince Michael II - nicknamed "Blanket" - was adopted in 2002.
"Michael is in bad shape," a pal told The ENQUIRER. "He has respiratory problems, heart palpitations, is losing weight rapidly and is now worried that he may have contracted the killer flesh-eating disease. He has begged Janet to look after the kids."
The health crisis developed following Michael's latest cosmetic surgery, which also included cheek implants, divulged his pal. The fear is that the stitches on his neck became infected, giving him a severe staph infection - resistant to conventional antibiotics.
"He's been visiting a Beverly Hills clinic receiving antibiotics through an intravenous drip," disclosed the pal. "Michael has difficulty walking, often has to use a wheelchair and spends most of his days in bed, at the doctor's office or in the bathroom.
"His sole companion is his longtime nanny Grace Rwaramba. Michael knows Grace loves the kids, but doesn't think she can cope with them on her own.
"Debbie, mother of Prince and Paris, has been out of the children's lives and only sees them rarely. Janet sees them all the time and is more like a mother to them than anyone.
"Family members have offered to help, but he's afraid of the children being exploited. The only family member he trusts is Janet."

Michael Jackson is dying - and has only six months to live, pals fear.
Sadly, the desperately ill star is bedridden much of the time, and his electrifying stage performances are a thing of the past, reveal sources.
Incredible reports surfaced in late December that Jackson is battling a rare, deadly genetic disorder and requires a lung transplant. Although the singer denied the reports, The ENQUIRER was told by sources that his health is deteriorating rapidly because of a crippling addiction to painkillers and alcohol.
A source close to the star divulged: "It's tragic. His condition is just so far gone, I'd be surprised if he lasts six months.
"Painkillers and booze have caught up with him. The only way he was able to cope with the stress of sex scandals and his roller-coaster life was to mask the pain with substance abuse.
"But the end result is an addiction that will kill him."
King Bizarro's dependency on drugs "dates back to when his hair was burned making a Pepsi commercial," said the close source. Twenty-five years later, "the drugs simply have ravaged him."
Michael's in "really, really bad shape," claims the family friend. "He's been sick for six to eight months and he just keeps getting worse.
"His muscles and lungs are deteriorating, and he's bedridden much of the time. He can walk, but not for very long.
"On good days, Michael is able to take his kids to a bookstore. But he's never out for more than an hour. He has very little strength...It's ultimately in God's hands."
Michael has been diagnosed with a rare genetic condition called alpha-1 antitrypsin deficiency that can cause lung and liver disease, according to Ian Halperin, author of a new Jackson biography.
And medical experts warn the reclusive singer's longtime addiction to morphine and the potent painkiller Demerol can make the condition devastatingly destructive.
Halperin said a friend of the star told him: "Michael wants to have the lung transplant, but because of other illnesses he's fighting, he's too weak to undergo such a major procedure. He's taking one painkiller after another."
Halperin claims that Jackson, a nonsmoker, is battling emphysema, and is also suffering from chronic gastrointestinal bleeding.
"It's the bleeding that could kill him," said the Canadian writer.
But a spokesman for Jackson vehemently denies the star is ill, saying: "Mr. Jackson is in fine health" and is in negotiations for a concert tour and TV projects.
Pulmonary specialist Dr. Len Horovitz, who once treated Michael for dehydration in 1999 at New York's Lenox Hill Hospital, has been following accounts of his lung problems.
"If Michael Jackson has alpha-1 antitrypsin deficiency, he needs to be very careful," Dr. Len Horovitz told The ENQUIRER.
"The disease produces an early-onset emphysema, and the liver can also be affected. If Jackson has the disorder, he also has to be very careful about taking sedatives and narcotics - such as morphine or pain pills - because they can greatly aggravate the problem.
"They don't actually cause the condition, but because they can depress breathing, they can be very dangerous.
"In advanced stages, a lung transplant may be necessary. But if Jackson needs a lung transplant, it means he's had so much damage he's not even a candidate for preserving what's left."
"Michael's fighting for his life," the close source concluded. "And it doesn't look good."

The King of Pop is on his deathbed, claims a shocking new report. Michael Jackson is said to be suffering from a genetic disease called A1AD — alpha-1 anti-trypsin deficiency — which has left him blinded in his left eye and so weak he is barely able to speak.
Jacko's biographer, Ian Halperin, tells Britain's Daily Express, "He needs a lung transplant, but may be too weak to go through with it. He also has emphysema and chronic gastrointestinal bleeding, which his doctors have had a lot of trouble stopping. It's the bleeding that's the most problematic part. It could kill him."
On his Web site Halperin added, "A close friend of Jackson told [me] the singer is scared he won't live longer than six months."
